Sildenafil citrate
Dosage/Direction for Use
Adult 50 mg approx 1 hr before sexual activity. May be increased to 100 mg or decreased to 25 mg. Max of 100 mg. Max recommended dosing frequency is once per day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to sildenafil, soya, peanut, Ponceau 4R. Co-administration w/ nitric oxide donors (eg, amyl nitrite) or nitrates. Patient w/ loss of vision in 1 eye because of non-arteritic anterior ischaemic optic neuropathy; patient for whom sexual activity is inadvisable (eg, patients w/ severe CV disorders eg, unstable angina or severe cardiac failure). Severe hepatic impairment, hypotension (BP <90/50 mmHg), recent history of stroke or MI & known hereditary degenerative retinal disorders eg, retinitis pigmentosa.
Special Precautions
Consider CV status of patients prior to treatment initiation. Reports of serious CV events. Use w/ caution in patients w/ anatomical deformation of the penis (eg, angulation, cavernosal fibrosis or Peyronie's disease), or in patients who have conditions which may predispose them to priapism (eg, sickle cell anaemia, multiple myeloma or leukaemia). Reports of visual defects & cases of non-arteritic anterior ischaemic optic neuropathy. Patients w/ bleeding disorders or active peptic ulceration. Concomitant use w/ other treatments for erectile dysfunction; α-blockers. Co-administration w/ ritonavir is not advised. Rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect ability to drive & use machines. Patients w/ hepatic impairment & severe renal impairment. Not indicated for use by women. Not indicated for individuals <18 yr.
Adverse Reactions
Headache, flushing, dyspepsia, visual disorders, nasal congestion, dizziness & visual colour distortion.
Drug Interactions
Reduced clearance w/ CYP3A4 inhibitors (eg, ketoconazole, erythromycin, cimetidine). Increase in Cmax & plasma AUC w/ ritonavir; saquinavir. Modest increases in plasma levels w/ grapefruit juice. Potential to have serious interaction w/ nicorandil. Potentiated hypotensive effects of nitrates. Symptomatic hypotension w/ α-blocker.
